Services Description
Your clinical record is maintained by our staff and contains information regarding assessments, progress notes, medications and treatments.
Your records are preserved and are kept strictly confidential.They are protected against loss, destruction, tampering or unauthorized use.
Certain parts of the medical record will be left in your home to ensure that any/all copies of your health information remain protected and aren’t disclosed to anyone other than your caregivers and physician. Please ensure their availability for our staff as they will be required to review recently added information or to record new information for other staff.
Our notice of Privacy Practices included in this booklet describe how your protected information may be used by us or disclosed to others, as well as how you have access to this information.
